Dr. Chan is a Delta Dental Premier provider. As a service to our patients, we work with most dental insurance programs, including Premier and PPO plans, both in-network and out-of-network, as well as non-managed care and traditional indemnity plans.

We are not part of managed care networks.

How Insurance Processing Works

Our accounting team prepares and submits the necessary claim forms for your dental benefits. We also offer pre-treatment estimates so you can better understand expected coverage before treatment begins.

Please note that your dental policy is an agreement between you and your insurance carrier. Coverage details, frequency limits, annual maximums, and exclusions vary by plan.

Financial Responsibility and Coverage Differences

Insurance companies often use their own fee schedules, which may differ from our usual and customary fees for similar services. Because of that, estimated coverage can vary.

You are responsible for your full patient balance if insurance pays less than anticipated.

Any unpaid portion becomes the patient responsibility. We will review the claim outcome with you and help clarify next steps.

Do you accept plans other than Delta Dental?

Yes. We work with many PPO and traditional plans and submit claims to most carriers.
